THE WINNING SOLUTION
The winning solution presented by Team Light Blue introduced the ISAR tool, developed at McGill’s St. Mary's Hospital, as the "seventh vital sign" to address ED access block, particularly for seniors. The ISAR tool consists of six screening questions that identify older patients at risk of adverse outcomes, helping determine whether they should be admitted or discharged. By streamlining decision-making, ISAR helps reduce ED length of stay and revisit rates while optimizing consults with social workers or occupational therapists.
